Registration procedure

The forms are available in several formats Please click on the appropriate form (Men’s or Women’s), print a copy, complete and either return with your payment to: LCFC, 392 Chelsea Rd, Kingston, ON, K7M 3Z9 or give completed form and payment to your team rep. Deadline for registration is April 30th. For Recreational teams it will be on a first come first serve basis.
